Procedures for Communicating Medical Conditions to Captains, Coaches and Volunteers

 

1. The relevant medical conditions for players under the age of 18 years are collected by the Secretary from the Membership Form. The Secretary will inform the Welfare Officer of the medical condition of any such player in order to ensure that the Welfare Officer may seek relevant advice and inform the Club of any precautions to be taken when the player participates in matches or net sessions.

2. The Secretary, as advised by the Welfare Officer, will provide to each captain and, as appropriate, coach or volunteer supervising a (Academy) match or net session a confidential note of the relevant medical condition of any such player together with instructions for the taking of any medicaments and contact details of parents/guardians.

3. The Secretary, as advised by the Welfare Officer, will update such confidential notes as and when required.

4. All such information on medical conditions will be maintained and communicated only with the consent of the player and/or parent/guardian and in accordance with data privacy considerations.
